Amidst the hoopla and chaos of this year’s HIMSS conference in Orlando, we introduced a new feature for our cloud-based RightPatient biometric patient identification solution: the RightPatient Smart App. This is kind of a big deal for the healthcare industry because the RightPatient Smart App has the power to turn any smartphone or tablet into a powerful patient recognition device.

As we have written about before, increased recognition of the critical importance of accurate patient identification for patient safety has played an important role in our own research and development of the RightPatient cloud biometric patient identification solution.  I don’t think I’m alone in saying that most patients see patient identification as the part of our healthcare experience that starts with sitting in front of a registrar at a healthcare facility so they can obtain our insurance information and make sure we are who we claim to be.

However, anyone who has spent time as a patient in a healthcare environment knows that most medical facilities don’t stop with establishing accurate identification at the point of registration. You may have your ID checked before medication disbursement, prior to the administration of a medical procedure, or perhaps just before surgery. This is important for patient safety, and to reduce the risk of adverse events from wrong patient procedures. 

The problem is that many patient identification mistakes are still regularly made across the healthcare industry. This can cause irreparable harm to patients and providers in many cases. Fortunately, we provide the most innovative technology in the market to solve this problem.

Patient identification is of the utmost importance for a doctor. It helps ensure that the patient’s identity matches the treatment or service. Keep in mind that the proper procedure for patient identification is for your staff to ask patients to state their name and date of birth. Identification issues might occur when patients have hearing issues, so it is wise to be on top of this problem.

Patient Data

Recording patient data can help your practice and the medical industry as a whole. Understanding a patient’s medical history and tracking a patient’s health, medications, and lifestyle helps doctors come up with a customized plan to reduce the chance of errors. This type of data helps establish patterns for clients and may even help you reevaluate a plan to provide what your patients need the most.

Track Record

Patients should be aware of a doctor’s track record so that they can make an informed decision. This can be done by searching for the doctor’s name on the state’s medical licensing board website. There may be some information on the doctor and his or her track record. It might be a good idea to type in the doctor’s full name and location in a search engine to find out more information about the doctor. A patient can rest easy knowing that a doctor’s track record is clean, and it should make it easy for the client to trust you.

Education

Most state medical licensing boards have information regarding a doctor’s education, but this is not always the case. There are some instances when the state’s licensing board may not be updated. It might be a good idea to check out sites like UCompareHealthcare to learn about a doctor’s education. You can even check out the rest of the staff’s education. If you wanted to look up a nurse who may have participated in an online msn program, for example, you could look him or her up to see what their record and education history looks like and be more assured that they are the right nurse for you. Patients should be able to find out about the medical professionals that that they see pretty easily. This helps them feel more comfortable, and it makes it easier to make a decision about making you the doctor or nurse that they request on a regular basis.

All this information matters, and it is vital in building the right relationship. It also makes a difference in the type of care you provide to your patients.

We work hard throughout the year to help our community stay informed of the latest news and information on the state of patient identification in healthcare. Our perspective is that the future of patient ID is the patient photo, and with good reason. The ECRI recently recommended that healthcare organizations use more standard means of patient identification, which should include patient photos with their electronic health records (EHR). Understanding that accurate patient ID in healthcare affects so many more downstream activities and is widely considered to be the “big bang” of effective and safe patient care, the urgency for hospitals and healthcare organizations to adopt more secure patient identification technology has never been stronger.

Healthcare providers should take note however that not all biometric patient identification solutions are equipped to address the challenges and complexities of ensuring ID accuracy across the entire care continuum which now includes a multitude of new touchpoints such as connected health devices, patient portals, telemedicine, home health, and more. Investing in a patient identification solution that simply provides the ability to accurately identify an individual when they are physically present at a medical facility is now considered short-sighted. Healthcare providers should now consider adopting patient ID technology that is easily scalable, and has the flexibility to capture and store a patient’s photo for accurate identification during any encounter along the care continuum.

In 2016, we wrote extensively about the impact of accurate identification on patient safety including several posts that extrapolate on the imperatives of capturing photos as part of the ID process. 2016 is a wrap. We observed a few positive advancements to improve patient identification in healthcare, but overall we remain concerned that the topic is often skirted in favor of bolder, more splashy initiatives (e.g. – MACRA, Blockchain, interoperability) which always seem to garner more attention. No doubt that these are important initiatives in the healthcare industry but as we have said many times before — accurate patient identification in healthcare arguably should have been the first problem solved before we tackled these other projects. However, factors at play make it perhaps one of the most difficult and complex healthcare issues to solve from a logistical, political, economical, privacy, and health data exchange perspective.

It is a well known fact that inaccurate or incomplete data within a patient’s medical record can be a catastrophic risk to patient safety, not to mention a serious hospital liability. As a result, many hospitals and healthcare organizations across the industry are closely examining the integrity of their health data and taking steps to clean it, most by using third party probabalistic and deterministic de-duplication matching algorithms (often directly from their EHR providers) that search and identify possible duplicates for an automatic or manual merge.

Improving patient data integrity in healthcare requires a multi-layered approach that addresses both data matching and more accurate patient identification.

Several key players in the healthcare industry including CHIME, AHIMA, HIMSS, and major EHR providers are beating the drum to improve patient identification and patient data matching, all important catalysts for the push to improve patient data integrity.

If you are a hospital or healthcare organization that is knee deep in the middle of a health IT initiative to help increase patient data integrity (especially in the context of prepping for participation in a local or regional health information exchange), you may want to stop and reassess your strategy.  The rush to cleanse “dirty data” from EHR and EMPI databases is often addressed by relying on an EHR vendor’s de-duplication algorithm which is supposed to search and identify these duplicate medical records and either automatically merge them if similarity thresholds are high, or pass them along to the HIM department for further follow up if they are low. 

This could be a very effective strategy to cleanse an EMPI to ensure patient data accuracy moving forward, but is it enough? Is relying on an EHR vendor’s de-duplication algorithm sufficient to achieve high levels of patient data integrity to confidently administer care?  It actually isn’t. A more effective strategy combines elements of a strong de-duplication algorithm with strong patient identification technology to ensure that patient data maintains its integrity.

Duplicate Medical Record Rates are Often Understated

The industry push for system-wide interoperability to advance the quality and effectiveness of healthcare for both individuals and the general population has been one of the main catalysts motivating healthcare organizations to clean and resolve duplicates but it also has revealed some kinks in the data integrity armor of many different medical record databases. Most hospitals we speak with either underestimate their actual duplicate medical rate, or do not understand how to properly calculate it based on the actual data they can access.  An AHIMA report entitled “Ensuring Data Integrity in Health Information Exchange” stated that:

“…on average an 8% duplicate rate existed in the master patient index (MPI) databases studied. The average duplicate record rate increased to 9.4% in the MPI databases with more than 1 million records. Additionally, the report identified that the duplicate record rates of the EMPI databases studied were as high as 39.1%.”

“High duplicate record rates within EMPI databases are commonly the result of loading unresolved duplicate records from contributing MPI files. EMPI systems that leverage advanced matching algorithms are designed to automatically link records from multiple systems if there is only one existing viable matching record. If the EMPI system identifies two or more viable matching records when loading a patient record, as is the case when an EMPI contains unresolved duplicate record sets, it must create a new patient record and flag it as an unresolved duplicate record set to be manually reviewed and resolved. Therefore, if care is not taken to resolve the existing EMPI duplicate records, the duplicate rate in an EMPI can grow significantly as additional MPI files are added.”

Clearly, the importance of cleansing duplicate medical records from a database cannot be understated in the broader scope of improving patient data integrity but relying on an EHR vendor’s probabilistic matching algorithm as the only tool to clean and subsequently maintain accurate records may not always be the most effective strategy. Instead, healthcare organizations should consider a multi-layered approach to improving patient data integrity beyond relying exclusively on an EHR vendor’s de-duplication algorithm. Here’s why.

Why Patient Data Integrity is a Multi-Layered Approach

Often not clearly explained to healthcare organizations, EHR de-duplication algorithms allow end users to set matching thresholds to be more or less strict, which comes with trade-offs. The more strict the threshold is set, the less chance of a false match but the higher chance of a false reject. The less strict the algorithm is set, the lower the chance of a false reject but the higher the chance of false acceptance.

Translation: Often times hospitals who say they have a low duplicate medical record rate might have a strict false acceptance rate (FAR) threshold setting in their de-duplication algorithm. That may mean that there could be a significant amount of unknown duplicate medical records that are being falsely rejected. Obviously, this is a concern because these databases must be able to identify virtually every single duplicate medical record that may exist in order to achieve the highest level of patient data integrity.

So, what can healthcare organizations do to ensure they are not only holistically addressing duplicate medical record rates, but also adopting technology that will maintain high patient data integrity levels moving forward? One answer is to implement a stronger de-duplication algorithm that has the ability to “key” and link medical records across multiple healthcare providers on the back end, and deploying a technology such as biometrics for patient identification on the front end to ensure that not only is care attribution documented to the accurate medical record, but a provider has the ability to view all patient medical data prior to treatment. 

For example, many credit bureaus offer big data analytics solutions that can dig deep into a medical record database to better determine what identities are associated with medical records. These agencies are experts in identity management with access to sophisticated and comprehensive databases containing the identification profiles for millions and millions of patients — databases that are reliable, highly accurate, and secure with current and historical demographic data.

Once data is analyzed by these agencies, they are able to assign a “key” to match multiple medical records for the same patient within a single healthcare organization and across unaffiliated healthcare organizations to create a comprehensive EHR for any patient. Offering a unique ability to augment master patient index (MPI) matching capabilities with 3rd party data facilitates more accurate matching of medical records across disparate health systems and circumvents the problem of MPIs assigning their own unique identifiers to individual patients that are different than unaffiliated healthcare organizations that have their own MPI identifiers.

Benefits of using a third party big data analytics solution that has the ability to “key” medical records for more accurate patient data matching at a micro level include:

  • More accurate identification of unique patient records resulting in a more complete medical record and improved outcomes
  • Prevention of duplicate medical records and overlays at registration reduces the cost of ongoing MPI cleanups
  • Medical malpractice risk mitigation 
  • Reduced patient registration times
  • The ability to more accurately link the most current insurance coverage patient information for more accurate billing

On the marco level, benefits include: 

  • Positive patient identification for eligibility verification, billing, coordination of benefits, and reimbursement
  • Improved care coordination
  • Information and record keeping organization 
  • Linkage of lifelong health records across disparate healthcare facilities
  • Aggregation of health data for analysis and research
  • Accurately aggregating patient federated data via a HIE

Conclusion

We have long championed the idea that improving patient data integrity can never be achieved in the absence of establishing patient identification accuracy or relying on EHR vendor de-duplication algorithms as the single resource to clean an MPI database. Hospitals and healthcare organizations that are truly committed to cleansing duplicate medical records from their databases and preventing them from reoccurring through more accurate patient identification must consider deploying stronger front and back end solutions that have the ability to more comprehensively identify and resolve these dangers to patient safety. Why not leverage the clout and reach of these big data analytics solutions to more effectively improve patient data integrity instead of putting all of your eggs in an EHR vendor’s de-duplication algorithm?

Novant Health Uses Iris Biometrics to Identify Unknown Patient

It’s a familiar case. An unconscious or unknown patient arrives in the ER without any identification leaving clinicians to administer care in the absence of any medical history to review. This presents a serious patient safety risk since treating an unknown patient without the benefit of securing their identity is dangerous and can be a huge liability. What if they are allergic to a certain medication? What if they have a pre-existing condition that must be considered prior to receiving any treatment?

Novant Health recently used the RightPatient iris biometrics patient identification system to identify an unknown, disoriented patient.

Since these cases are more often trauma related requiring immediate attention, clinicians must take a risk and administer care in the absence of any historical medical data. An obvious threat to patient safety and a situation that clearly raises liability, healthcare organizations have long sought to adopt technology that can instantly identify patients in these cases without the need for any demographic information. 

The staff at Novant Health decided to proactively implement an iris biometric identification system throughout their network as a means to secure accurate patient ID and ensure that patients, no matter what the circumstances, are kept safe throughout the care continuum. Although adopting a biometric patient identification system to identify unconscious or unknown patients wasn’t the sole reason that Novant implemented this technology, they knew that by choosing to use iris recognition as their primary biometric modality they would be able to quickly and accurately identify any patient in these circumstances without having to ask for an additional identification credentials (e.g. – D.O.B.).

Novant’s iris biometric patient identification system was recently put to the test when a disoriented, unknown patient arrived in the ER without any identification credentials. Novant staff quickly realized that they could take the patient’s photo with a RightPatient iris camera and if they had been previously registered in their Epic EHR database, the biometric patient identification system would recognize them and immediately pull up their medical record. Fortunately, the patient had previously been enrolled with the RightPatient system and their identity was instantly recognized after their photo was taken with the iris camera. A big relief to Novant staff since they were now able to not only access her medical history prior to treatment, but they were also able to quickly contact the patient’s relatives to inform them of the situation.

What often goes unnoticed is the impact that biometric patient identification solutions have on Health Information Management (HIM) departments. Often considered the “medical record gatekeepers” of the healthcare industry, HIM departments perform one of the most critical functions in the healthcare work flow by ensuring the safety of patients through medical record accuracy. HIM also helps to facilitate fast and efficient payments under strict time constraints for services rendered, and spend a lot of time correcting patient records because healthcare facilities want to be paid on the care provided to patients in a timely fashion.

A large part of medical record reconciliation is resolving duplicate medical records and overlays (when two patients medical histories appear on one medical record) which consumes Full Time Equivalents (FTE’s), and swallows up resources that could otherwise be spent on more value-added tasks that directly impact revenue cycle management and limits penalties.

Litigation is also an important point to stress. A patient’s chart must be able to withstand the scrutiny of a legal proceeding if a patient were to sue a healthcare facility lending even more importance to the work of HIM to ensure medical record accuracy. There is also the issue of reporting. The HIM department is directly responsible that medical records are accurate for quality reporting which has a direct impact on reimbursement and avoiding penalties imposed by the Center for Medicaid and Medicare Services (CMS) that range from readmissions to demonstrated improvement in patient outcomes. Keep in mind the new healthcare paradigm – stressing the quality vs. the quantity of services provided, a huge shift change that carries additional responsibility and an increased sense of urgency to ensure data accuracy at any healthcare facility.

Which brings us to the use of biometrics for accurate patient identification on the front end.

Healthcare facilities that have invested in deploying biometrics for accurate patient identification to prevent duplicate medical records and overlays on the front end are seeing the trickle down benefits to HIM departments, specifically the fact that they are spending less time reconciling duplicates and overlays and more time on coding, revenue cycle management, and reporting. It should be noted however, that biometric patient identification solutions built with search capabilities based on “one-to-many” matching types are the only solutions available that can truly prevent duplicate medical records, fraud, and medical identity at the point of registration. Do your homework before selecting a vendor, not all offer this type of back end matching capability. 

Why is it important to reduce HIM FTEs spent on reconciliation of duplicate medical records and overlays? As noted earlier, many hospitals have expanded responsibilities vis-à-vis Meaningful Use, EHR implementation, and meeting Affordable Care Act requirements, and it has become disadvantageous to continue devoting any time at all to duplicate medical record and overlay reconciliation. Biometric patient identification solutions open the door to re-allocation of HIM FTEs to more critical functions such as coding, reimbursement, and reporting. Simply put, implementing biometrics during patient registration is opening the door for HIM departments across the industry to provide a larger and more productive support role to meet the shifting sands of reimbursement and address the need to move towards quality vs. quantity of care.

Hospitals should be actively seeking to deploy patient matching and patient identification technologies that eliminate barriers (e.g. duplicate medical records, overlays) and maximize HIM productivity to shift FTEs away from continuous master patient index (MPI) cleanup and more towards coding, quality review, reimbursement, and other areas. Many hospitals are already re-aligning their HIM departments in the wake of EHR implementation, and we expect to see more of the same for those using biometrics for accurate patient identification. 

What other ways can the use of biometrics for patient identification reduce HIM FTEs?
